EZ Mobile Application Privacy Policy for Patients

Overview

EZ takes very seriously its obligation to protect the confidentiality of your personal information.

EZ’s mobile applications for patients, including the EZ Healthcare APP for iOS and Android, are intended to connect you to information received from Your Healthcare Provider (YHP) in order to provide you secure, mobile access to those systems and to your health information.

This App is used primarily to allow users to display appointment information, prescriptions, physician notes and laboratory results on their personal mobile devices as authorized by them and their providers.

Some of the health data we have or may collect as part of this EZ Healthcare software application is protected by HIPAA. Read our HIPAA Notice of Privacy Practices.

All transmission and storage methods are HIPAA compliant using a variety of technical safeguards, supporting Transport Layer Security (TLS)/Secure Sockets Layer (SSL) certificate technology and data encryption.

This technology or App does not request access to other device data or applications, such as your phone’s camera, photos, or contacts.

This technology or App does not share the collected data with your social media accounts, such as Facebook.

This Privacy Policy

This Privacy Policy describes how EZ mobile applications for patients (our “Applications”) use, store and transmit information and data collected from you, how the app processes your data when displayed on your device, and how EZ processes your personal information received from Your Healthcare Provider (YHP). By using our Applications to connect to your healthcare information, you consent to this Privacy Policy. EZ may modify this Privacy Policy at any time effective upon its posting. Your continued use of our Applications constitutes your acceptance of this Privacy Policy and any updates. Your use of our Applications is subject to the of the applicable Applications’ End User License Agreement.

Information Collected from You and Stored on Your Device

EZ does not sell, share, or license any information that it may collect from you from using our Applications.

Our Applications do not store any of your personal information on your device. Our Applications do not send any personal information back to EZ without your permission. Any personal information sent to EZ through use of Our Application is protected pursuant to HIPAA guidelines in regards to storage and transmission.

The information that is provided to you during the display processes is temporary and is released and deleted as you close the APP. It is not retained on your device following closure.

EZ attempts to minimize the amount of your personal or health information stored or retained on your device. Nevertheless, our Applications may:

  • Create encrypted identifiers to identify target healthcare providers you have authorized.
  • Temporarily store your personal information in memory or on the device while you use our Applications.
  • Share some limited amount of information for purposes of troubleshooting and error correction directly or indirectly with EZ with your consent.
  • In addition, in order to provide you certain features, our Applications may request information from servers and systems owned or operated by EZ and those servers and systems may record technical information about that request such as an IP address.

Information from Your Healthcare Provider

EZ does not sell, share, or license any information that it may collect from Your Healthcare Provider from you using Our Applications.

Who Has Access to Your Information?

When your information is provided to EZ as part of the process, your information WILL NOT be shared with any third parties.

Any personal information EZ Information Technology (IT) staff has access to as part of their role is subject to HIPAA protection.

How We Protect Your Information

We use a combination of process, technology and physical security controls to help protect your information from unauthorized access, use, or disclosure, but remember that no method of transmission over the Internet, or method of storage, is 100% secure.

To use Our Applications, you must have received an account from a healthcare provider who uses EZ’s software and given consent for your healthcare provider to provide access to you or your designated representative through this app. You are further required to activate your device using a one-time activation Personal Identification Number (PIN). We save the specific device information being used and do not allow other devices access without being specifically authorized.

Your use of Our Application is also subject to Your Healthcare Provider’s privacy policy. You understand that while connected, or attempting to connect, to a healthcare institution’s system, the healthcare institution’s policies may result in your provider transmitting or storing data transmitted through Our Application. Please contact your healthcare institution if you have any questions about their policies or terms.

How We Protect Your Personal Information

The security of your information and data, at all times including while using Our Applications is very important to us. Our Applications employ a variety of technical safeguards to protect the confidentiality, integrity, and availability of your personal information including supporting Transport Layer Security (TLS)/Secure Sockets Layer (SSL) certificate technology and encryption.

In addition, healthcare providers with whom you connect may use a variety of physical, administrative and technical measures to protect your personal information.

EZ will protect consumers in the event of a breach by complying with the HIPAA Disclosure Policy.
